1)Received a Infringement of personal privacy of a court ruling.(Must Required)

2)The data and time when the alleged infringement was discovered.

3)A description of the specific material that is alleged to be Infringement of personal privacy.

4)The nature of the alleged infringement.

5)The online location where the allegedly Infringement of personal privacy material is found.

6)Signed by the Infringement of personal privacy owner or the Infringement of personal privacy owner's duty authorised agent.(Must need certificate of a court ruling)

7) a statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the Infringement of personal privacy owner, its agent, or the law; and

8)a statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
